Hy-Vee, Inc. is excited to partner with Submittable, a grants management platform, to bring you a streamlined application process when submitting donation requests. On this platform, you can submit, manage, and track your giving request applications and communicate directly with Hy-Vee regarding your request.
How it Works:
To request a donation or sponsorship from Hy-Vee, follow the below steps after reviewing our Charitable Giving Guidelines:
- Step 1: Discuss your request and proposed amount with your local Hy-Vee store or location to confirm their support before applying.
- Step 2: Calculate the estimated total of your request (monetary and/or in-kind value) and select the application that corresponds to that amount
- If you’re unsure which tier your request falls under, discuss your request directly with the Hy‑Vee store or location before submitting. Otherwise, choose the lower amount or the amount closest to what’s been supported in the past—unless pre-approved for a higher amount.
- Step 3: Once your application is submitted, it will be reviewed by the Hy-Vee location you select in the application form. Depending on the requested amount, additional reviewers and processing time may be required. You can check the status of your request anytime in your Submittable account.

Before submitting a request, please connect with your local Hy-Vee point of contact to discuss your event or need and confirm a donation amount. Requests are much more likely to be considered if there’s already an agreement in place with a Hy-Vee representative.
This form should be used only for requests with a total value (cash and/or in-kind) not exceeding $499.
If your request:
- Falls between $500 and $2,499, please use the Hy-Vee Sponsorship and Donation Request form ($500 - $2,499)
- Exceeds $2,499, please use the Hy-Vee Sponsorship and Donation Request form ($2,500 and above)

Important: Requests over $499 require additional review and processing time. Without a prior agreement with your Hy-Vee contact, these higher-level requests are unlikely to be approved. Please ensure your submission aligns with what has already been discussed and agreed upon.

Note: Requests at this level require additional review and processing time. Submissions without prior discussion and agreement with a Hy-Vee contact are unlikely to be approved.
Before submitting a request at this level, you must first connect with your Hy-Vee point of contact to discuss your event or initiative and agree on a sponsorship or donation amount. Requests above $2,500 require additional review and are only considered if alignment with a Hy-Vee representative has already been established.
Use this form if your total request value (cash and/or in-kind) exceeds $2,500.
If your request:
- Does not exceed $499, please use the Hy-Vee Sponsorship and Donation Request form (up to $499)
- Falls between $500 and $2,499, please use the Hy-Vee Sponsorship and Donation Request form ($500 - $2,499)
